Presentation Synopsis:
On October 17, 2017 a fatal ammonia release occurred at the Fernie Memorial Arena in British Columbia. This presentation will examine the incident itself, the technical aspects of the failure, the analysis completed during the investigation, and the root causes of the failure.
Presented by: Greg Scrivener, P. Eng.
Greg Scrivener was a member of the Technical Safety BC investigation team that investigated this incident. He participated throughout the entire investigation as an integrated team member and refrigeration technical expert. Greg is currently Lead Refrigeration Engineer and a Partner at Laporte Engineering where he leads the North American refrigeration engineering team.
